Targeted Earlier Intervention Caseworker – Weave Woolloomooloo

We are seeking a Targeted Earlier Intervention Caseworker to join our Weave Woolloomooloo team and provide intensive wrap-around case management for families with children aged 0-12 years and young people aged 12 - 25 years, predominantly from the Woolloomooloo, Darlinghurst and Kings Cross area. Case Management includes but is not limited to therapeutic and practical assistance, housing support, AOD and mental health support, information and referral, education assistance, access to support services, activities, groups, projects, and events.
